How to replace sim card ?
First:- you have go to your operator outlet/operator office then ask the process of sim replacement and then you have completed all the process whose said by company guidelines and then apply for sim replacement.
Carry documents while sim replacement time?
You have to carry these documents (anyone):-
1) aadhaar card
2) pancard
3) driving licence
4) voter identity card
5) passport
Or valid address or photo identity card only for India citizens.

The charges of sim replacement is different and based of operator service so you have go to operator outlet or operator point for know charges.
